L'utilizzo di MDMA nella psicoterapia nei pazienti con PTSD : review e meta-analisi

Abstract

La 3,4-metilenediossimetanfetamina è una molecola con proprietà psicoattive storicamente usata in campo medico e ricreativo. A causa dei suoi effetti collaterali, la molecola è stata messa al bando in diversi Paesi. Tuttavia, gli studi hanno testato l'efficacia dell'MDMA come coadiuvante in psicoterapia per il trattamento del del disturbo da stress post-traumatico. La tesi esplora lo stato dell'arte della MDMA nella letteratura scientifica nel trattamento del PTSD sia da un punto di vista qualitativo sia quantitativo, tramite una meta-analisi. L'obiettivo della meta-analisi è stato quello di verificare l'efficacia dell'MDMA nel trattamento psicoterapeutico del PTSD in disegni longitudinali in cui un gruppo sperimentale assumeva il farmaco e un controllo un placebo. L'effect size utilizzato è stato la differenza media standardizzata con indice ∆ di Glass. La raccolta dati è avvenuta a Maggio 2023 sui database EMBASE, Pubmed, e Google scholar. Gli articoli inclusi nella meta-analisi sono stati 11, e i risultati indicano un valore ∆ = -0.95 95% CI [-1.13; -0.77] a favore del gruppo sperimentale, con eterogeneità nulla (I^2 = 0%); non sono stati individuati predittori statisticamente significativi. La tesi ha mostrato l'importanza di indagare sul tema in quanto la letteratura e lo studio meta-analitico presentato mettono in luce gli effetti positivi dell'MDMA nel trattamento del PTSD.

3,4-methylenedioxymethamphetamine is a molecule with psychoactive properties historically used in both medical and recreational fields. Due to its side effects, the molecule has been banned in several countries. However, studies have tested the efficacy of MDMA as an adjunct in psychotherapy for the treatment of PTSD. This thesis examines the current state of MDMA in scientific literature regarding PTSD treatment from both a qualitative and quantitative perspective through a meta-analysis. The goal of the meta-analysis was to verify the efficacy of MDMA in psychotherapeutic treatment of PTSD in longitudinal designs where one experimental group receives the drug and a control group receives a placebo. The effect size used was the standardized mean difference with Glass's Δ index. Data collection took place in May 2023 from the EMBASE, Pubmed, and Google Scholar databases. Eleven articles were included in the meta-analysis, and the results indicate a value Δ = -0.95 95% CI [-1.13; -0.77] in favor of the experimental group, with no heterogeneity (I^2 = 0%), and exploratory analyses did not identify any statistically significant predictors. The thesis highlighted the importance of investigating this topic as the literature and the presented meta-analytic study show positive effects of MDMA in the treatment of PTSD.
