Abstract: This paper aims to review the literature regarding Acute Kidney Injury (AKI) with the goal of defining its canons and possible future perspectives. The definition and grading of AKI are based on the current guidelines of the International Renal Interest Society, an international society of small animal nephrology, with the aim of outlining its limitations and knowledge on the subject. Included in the paper is a brief excursus of renal anatomy and physiology with the aim of giving a knowledge base in order to better understand the underlying pathological mechanisms. This is followed by an extensive summary of the etiologies of AKI, in which the aforementioned are divided into four main categories to better understand their contributions to the genesis of the pathology. Next, the paper converges in the analysis of an appropriate diagnostic procedure by analyzing the elements that to date are available to the clinical veterinarian for the diagnosis of AKI. Future diagnostic biomarkers that, to date are under experimental study to be validated in the diagnosis of AKI, are analyzed. In the last instance, the therapy of AKI is discussed, divided into pharmacological supportive therapy and renal replacement therapy. Relative to renal replacement therapy, the operation of the most modern dialysis systems differentiated by treatment modality is technically analyzed. The last chapter is devoted to the prognosis of the disease, divided according to the underlying etiologies and the type of treatment used.
Questo elaborato si prefigge di analizzare la letteratura in merito all’Acute Kidney Injury (AKI) con l’obiettivo di definirne i canoni e le eventuali future prospettive. La definizione ed il grading di AKI si baseranno sulle attuali linee guida dell’IRIS, società internazionale di nefrologia dei piccoli animali, con lo scopo di delinearne i limiti e le conoscenze in merito. Viene incluso nell’elaborato un breve excursus dell’anatomia e fisiologia renale con lo scopo di dare una base di conoscenza per poter comprendere al meglio i meccanismi patologici che sottendono la patologia. Seguirà poi un’ampia sintesi sulle eziologie causa di AKI, nella quale le suddette verranno ad essere suddivise in quattro categorie principali per meglio comprenderne i contribuiti alla genesi della patologia. L’elaborato convergerà poi nell’analisi di un iter diagnostico appropriato andando ad analizzare gli elementi che ad oggi sono a disposizione del veterinario clinico per la diagnosi di AKI. Verranno analizzati anche futuri biomarker diagnostici che ad oggi sono oggetto di studio sperimentale per poter essere validati nella diagnosi di AKI. Si passerà in ultima istanza a trattare della terapia di AKI, suddivisa in terapia di supporto farmacologico e terapia renale sostitutiva. Relativamente alla terapia renale sostitutiva verrà analizzato tecnicamente il funzionamento dei più moderni sistemi di dialisi differenziati in base alla modalità di trattamento. Infine vi sarà un capitolo dedicato alla prognosi della patologia, suddivisa in base alle eziologie che la sottendono e alla tipologia di trattamento utilizzato.
