dc.description.abstract | Background. The concept of WBP (wound bed preparation) has revolutionized the way to diagnose and correctly identify the best therapeutic path about the widespread clinical problem of difficult wounds. Starting from the modified TIME-H, the present Doctoral thesis focuses on an extended study with the aim of assessing the impact of skin lesions and soft tissues for the surgical patient. Materials and Methods: 38 patients were evaluated. The patients were classified according to the lesion examined, in particular among those who had an infectious or vascular etiology (SSTIs), and patients with surgical site lesions (SSI) and assigned to one of three categories prognosis: favorable (with healing expected within 12 weeks) (0-3A, 0-1B), intermediate (with healing expected over 12 weeks) (4-6A, 2-4B) and uncertain healing (7- 8A, 5-8B). Results: At the end of the observation period, this translational study established the healing prediction rate among the studied lesions: the surgical site lesions presented the highest percentage of predictivity (88%), followed by the mixed etiology (72%) and the infectious/vascular injuries (63%). Conclusion: This modified-TIME-H can be considered as a versatile and useful scoring tool that should be used in daily clinical practice for the study and treatment of chronic wound diseases. | en_US |
dc.description.abstract | Background. Il concetto di WBP (preparazione del letto della ferita) ha rivoluzionato la metodica per diagnosticare e identificare correttamente il miglior percorso terapeutico a proposito del problema clinico, molto diffuso, delle ferite difficili. A partire dal protocollo modificato TIME-H, la presente tesi di dottorato si concentra su uno studio esteso con l'obiettivo di valutare l'impatto delle lesioni cutanee e dei tessuti molli per il paziente chirurgico. Materiali e metodi: sono stati valutati 38 pazienti. I pazienti sono stati classificati secondo la lesione esaminata, in particolare tra coloro che hanno avuto un'eziologia infettiva o vascolare (SSTI) e pazienti con lesioni del sito chirurgico (SSI) e assegnate a una delle tre categorie di prognosi: favorevole (con guarigione prevista entro i 12 anni Settimane) (0-3a, 0-1b), intermedio (con guarigione prevista per 12 settimane) (4-6a, 2-4b) e guarigione incerta (7- 8A, 5-8b). Risultati: Alla fine del periodo di osservazione, questo studio traslazionale ha stabilito il tasso di previsione della guarigione tra le lesioni studiate: le lesioni del sito chirurgico hanno presentato la massima percentuale di predittività (88%), seguita dall'eziologia mista (72%) e le lesioni infettive / vascolari (63%). Conclusione: lo score TIME-H modificato può essere considerato come uno strumento di punteggio versatile e utile, che dovrebbe essere utilizzato nella pratica clinica quotidiana per lo studio e il trattamento delle malattie croniche delle ferite. | en_US |
