Abstract: INTRODUZIONE. La cefalea è una tra le più frequenti patologie del sistema nervoso, ma, nonostante ciò, in Europa continua ad essere sottostimata, a volte non viene diagnosticata e conseguentemente non trattata. Anche a livello di ricerca, nonostante la sua alta incidenza, la cefalea è ancora oggi trascurata. In questo studio ci si ipotizza di (Ipotesi 1) indagare se nei pazienti che soffrono di cefalea vi sia la presenza di neuroinfiammazione rispetto alle persone che non ne soffrono. Successivamente ci si pone di (ipotesi 2) verificare se nei soggetti cefalalgici vi sia la presenza di stati umorali alterati, come depressione, di disturbi d'ansia, somatizzazione etc., rispetto ai soggetti non cefalalgici. Inoltre, ci si pone di (ipotesi 3) analizzare se nei pazienti che soffrono di cefalea potrebbe esserci un'alterazione delle funzioni cognitive (in particolare le funzioni esecutive) e nella memoria, come segno di una modificazione celebrale, rispetto ai soggetti non cefalalgici, e infine ci si pone di (ipotesi 4) verificare, nei due gruppi, se l’assunzione di caffè, l’essere fumatori e praticare attività sportiva regolarmente possano influire sul dolore alla testa. METODO. Il seguente progetto di ricerca è iniziato a ottobre 2019 e si è concluso a gennaio 2021. Per questa ricerca sono state selezionate 17 persone che presentano spesso “mal di testa” (15 femmine e 2 maschi), andando a formare il campione sperimentale e altre 15 persone non cefalalgici (12 femmine e 3 maschi), formando così il campione di controllo; i soggetti selezionati hanno tutti un’età compresa tra i 18 e 55 anni. In questo progetto di tesi sono stati utilizzati i seguenti strumenti: Questionario anamnestico, Scala di autovalutazione S.C.L.-90R, PPG stress Flow di Biotekna S.r.l., alcuni subtest dell’ENB-2 e il Benton Visual Retention Test (forma C) RISULTATI. Per quanto riguarda la prima ipotesi, viene confermata in quanto è emersa una differenza staticamente significativa, i risultati suggeriscono un’alterazione della corteccia prefrontale nei soggetti cefalalgici, correlata ad una possibile neuroinfiammazione. Anche la seconda ipotesi viene confermata, i soggetti cefalalgici presentano maggiori punteggi alle scale ansia, depressione, ossessivo-compulsivo e somatica del test SCL-90R, confermando una comorbilità tra ansia e depressione già evidenziata in letteratura. Per quanto concerne la terza ipotesi, questa viene confermata in parte, evidenziando una differenza significa tra i due gruppi, ai subtest memoria di prosa differita e TMT-A dell’ENB2 e al test Visual Retention test Benton. Infine, la quarta ipotesi viene falsificata in quanto, risulta esserci una differenza significativa solo per l’assunzione di caffè
INTRODUCTION. Headache is one of the most frequent pathologies of the nervous system, but, despite this, in Europe it continues to be underestimated, poorly diagnosed and not treated as a consequence. Despite its high incidence, nowadays headache is still neglected even at the research level. In this study we tested numerous hypotheses. Hypothesis one investigates whether patients suffering from headache are affected by neuroinflammation compared to people who do not suffer from it. Subsequently, hypothesis two verifies whether headache subjects suffer from altered mood states, such as depression, anxiety disorders, somatization, etc., compared to non-headache subjects. Furthermore, hypothesis three analyze whether patients suffering from headache present an alteration of cognitive functions (executive functions specifically) and memory, as a sign of a cerebral modification, compared to non-headache subjects. Conclusively, hypothesis four tests whether high coffee intake, being a smoker and practicing sports regularly can affect head pain. METHOD. The following research project started in October 2019 and ended in January 2021. For this research, 17 people who often present "headaches" (15 females and 2 males) were selected, forming the experimental sample, while an additional 15 non-headache patients (12 females and 3 males), composed the control sample. The selected subjects aged between 18 and 55 years old. The following tools were used in this thesis project: Anamnestic questionnaire, Self-assessment scale S.C.L.-90R, PPG stress Flow by Biotekna S.r.l., specific ENB-2 subtests and the Benton Visual Retention Test (form C). RESULTS. As for the first hypothesis, a statically significant difference emerged between the headache group and the control one, suggesting an alteration of the prefrontal cortex in headache patients, linked to a possible neuroinflammation. The second hypothesis was also confirmed, since headache subjects showed higher scores on the anxiety, depression, obsessive-compulsive and somatic scales of the SCL-90R test, confirming a comorbidity between anxiety and depression already highlighted in the literature. The third hypothesis, was partially confirmed, highlighting a difference between the two groups in the subtests of the delayed prose memory and TMT-A of ENB2 and in the Visual Retention test Benton. Finally, the fourth hypothesis was falsified, as it appeared to be a significant difference between the headache group and the control one only for amount of coffee intake.
