Abstract: La malattia renale cronica (Chronic Kidney Disease,CKD) rappresenta un'importante causa di morbilità e mortalità nei gatti domestici, specialmente in età avanzata. La perdita di nefroni funzionanti associata alla CKD, condiziona l'omeostasi di numerosi soluti escreti principalmente nelle urine, compresi i fosfati. Il fosforo è un nutriente essenziale il cui assorbimento intestinale dipende principalmente dalla composizione della dieta. In condizioni fisiologiche, i reni svolgono un ruolo cruciale nel mantenimento dell'equilibrio del fosforo nell'organismo, in quanto regolano l'escrezione urinaria dei fosfati in base alla quantità effettivamente assorbita dall'intestino. Nei gatti affetti da CKD, l'assunzione di una quantità elevata di fosforo, come spesso si riscontra nelle diete di mantenimento, è stata associata all'insorgenza di disturbi minerali e ossei e alla progressione della malattia renale cronica. La riduzione dell'apporto alimentare di fosforo è una delle principali strategie della terapia nutrizionale che è un caposaldo del trattamento medico conservativo volto alla gestione del paziente felino con CKD. Numerosi studi presenti in letteratura scientifica suggeriscono il ruolo dell’eccessiva assunzione dietetica di fosforo come fattore di progressione del danno renale nella malattia renale cronica felina, ma è anche plausibile che possa rappresentare un fattore predisponente per l'insorgenza di danni renali anche negli animali sani, fino alla comparsa di CKD clinicamente manifesta. La tesi è strutturata descrivendo inizialmente l'omeostasi del fosforo in condizioni fisiologiche e il fosforo come nutriente. Successivamente, dopo una breve introduzione sulla malattia renale cronica nei gatti domestici, la tesi affronta le alterazioni dell'omeostasi del fosforo nella CKD felina, le conseguenze sul rene e sull'intero organismo e la relativa gestione nutrizionale. Infine, l'ultima parte affronta la questione dell'eccessiva assunzione giornaliera di fosforo altamente biodisponibile come fattore predisponente per l'insorgenza di CKD nei gatti adulti clinicamente sani.
Chronic kidney disease (CKD) is an important cause of morbidity and mortality in domestic cats, especially in old age. The loss of functioning nephrons associated with CKD, conditions the homeostasis of numerous solutes excreted mainly in the urine, including phosphates. Phosphorus is an essential nutrient whose intestinal absorption depends mainly on the composition of the diet. In physiological conditions, the kidneys play a crucial role in maintaining the phosphorus balance in the body, as they regulate the urinary excretion of phosphates based on the amount absorbed by the gut. In cats suffering from CKD, the intake of a high amount of phosphorus, as often found in maintenance diets, has been associated with the onset of mineral and bone disorders and the progression of chronic kidney disease. The reduction of the dietary intake of phosphorus is one of the main strategies of nutritional therapy which is a cornerstone of conservative medical treatment for management of the feline patient with CKD. Although existing evidence suggests that excess dietary phosphorus may contribute to the progression of kidney damage and fibrosis in feline chronic kidney disease, it is also plausible that it may play a role in the onset of kidney damage in this species. The thesis is structured by initially describing the homeostasis of phosphorus in physiological conditions and phosphorus as a nutrient. Subsequently, after a brief introduction on chronic kidney disease in domestic cats, the thesis deals with alterations in phosphorus homeostasis in feline CKD, the consequences on the kidney and the whole organism and the relative nutritional management. Finally, the last part address the issue of excessive daily intake of highly bioavailable phosphorus as a predisposing factor for the onset of CKD in healthy adult cats.
