Research in the field of Brain-Computer Interface (BCI) has exponentially increased over the past few years, demonstrating their effectiveness and application in various areas. This paper will discuss how it is possible to measure the engagement index of a user who interacts with a BCI through the ratio suggested by Pope (beta / (alpha + theta) starting from the EEG signal recording and showing how this index can be in relationship with the improvement of cognitive performance during neuromodulation. Specifically, the first part will deal with an excursus on arguments related to the Brain- Computer Interface and a brief discussion concerning the construct of audio-visual entrainment and its effect in the treatment of personality and behavior disorders or in improving of cognitive performance. In the second chapter, the applications of BCI in the field of neuro-feedback training aiming at increasing cognitive function will be explored. In this regard, the role of the engagement will be discussed, defining it as involvement and commitment that includes "perception - cognition - action - experience" during an interaction with a BCI, in order to achieve the optimal status and performance. Finally, in the third chapter will be reported the research conducted on a sample of 18 subjects who participated in some experimental studies carried out along the period comprised between 2015 to 2019 years at the Laboratory of Cognitive Psychology of the Department of Medicine and Surgery of Parma University. In these studies, a prototype called Neuro-Upper (NU) for the synchronization of brainwaves through audio-visual entrainment was tested demonstrating its value in treatment of several psychiatric disorders. For the present study, brainwave data of each session were extracted from all subjects who underwent treatment over the period completing a cycle of at least 55-75 sessions of 30 minutes, regardless of the outcomes showed by the clinical and neuropsychological evaluations before and after the treatment, such as the remission of the symptoms related to personality and behavior disorders and/or the increase of cognitive performances. The average engagement and arousal indices were then measured using the ratio devised by Pope (beta / (alpha + theta) ((beta high + beta low) / (alpha high + alpha low)), respectively to evaluate a potential significant relationship between the engagement index and the improvement of cognitive performance obtained following NU neuromodulation, while we expected a non-significant relationship with the arousal index. Statistical analysis confirmed both hypotheses, attributing a significant effect on the improvement of cognitive performances only for the relationship owing to the engagement. Finally, limitations and some future challenges that could allow a deepening and improvement of the hardware-software characteristics of NU and its use will be discussed.
