Abstract: RAZIONALE: I soggetti affetti da Disturbo Ossessivo Compulsivo di Personalità (DOCP) mostrano gradi variabili di disfunzionamento psicosociale. Infatti, le caratteristiche psicopatologiche nucleari del disturbo (eccessive rigidità e perfezionismo) potrebbero da un lato risultare funzionali in alcuni ambiti (es. lavorativo), dall’altro essere scarsamente adattative in ambito interpersonale. Inoltre, sebbene alcuni studi evidenzino un maggior funzionamento psicosociale in soggetti DOCP della comunità, campioni clinici con DOCP e comorbidità psichiatrica (in primis disturbi dell’umore) sembrano mostrare rilevante disadattamento in varie aree. Nessuno studio ha ad oggi chiarito se il disfunzionamento psicosociale sia specifico del DOCP, se invece rifletta una sua associazione con disturbi dell’umore, e se sia sotteso da peculiari meccanismi di processazione delle regole sociali. OBIETTIVO: Valutare il grado di funzionamento psicosociale in soggetti affetti da DOCP richiedenti trattamento psichiatrico (DOCP), soggetti non clinici con DOCP (DOCP-NC), controlli sani (CS) e pazienti affetti da Disturbi dell’Umore in remissione (DUr), ed esaminarne le risposte nel corso di un paradigma sperimentale in cui le regole sociali vengano rispettate e quindi violate (Cybershape). METODI: 40 DOCP, 41 DOCP-NC, 22 DUr e 81 CS hanno completato questionari indaganti il funzionamento psicosociale globale, relazionale e socio-lavorativo; le differenze tra gruppi sono state valutate tramite ANOVA univariata con test post-hoc di Bonferroni. In seguito, tutti i soggetti hanno partecipato a Cybershape, un esperimento al computer che propone due condizioni di scambio interpersonale: Fair Play (il soggetto gioca con altri giocatori che rispettano le regole), e Rule Violation (gli altri giocatori infrangono sistematicamente le regole del gioco). Dopo ogni condizione sperimentale i soggetti hanno riportato la sensazione di violazione delle proprie aspettative e la percezione che le regole venissero violate durante il gioco. Differenze tra gruppi in entrambe queste variabili sono state indagate tramite ANOVA a modello misto a misure ripetute, inserendo come fattore fra soggetti il Gruppo (DOCP, DUr, DOCP-NC e CS) e come fattore entro soggetti la Condizione sperimentale (Fair Play vs. Rule Violation). RISULTATI: I DOCP mostrano un funzionamento socio-lavorativo e relazionale inferiore rispetto a tutti gli altri gruppi, mentre i DOCP-NC non differiscono dai CS e mostrano un miglior funzionamento dei DUr. Come atteso, DOCP e DOCP-NC differiscono da CS e DUr sia per un maggior grado di perfezionismo sia di rigidità (p<.001); i DOCP-NC, tuttavia, mostrano rigidità inferiore rispetto ai DOCP. In Cybershape, dopo la condizione di Fair Play i DOCP riferivano una maggior sensazione che le loro aspettative fossero violate rispetto ai CS e ai DOCP-NC; tale sensazione era simile a quanto esperito nella condizione di Rule Violation (Gruppo x Condizione: F=.28, p=.6). I DOCP-NC riportavano, invece, una maggior violazione delle regole durante il gioco rispetto ai CS e ai DOCP, indipendentemente dalla condizione sperimentale (Gruppo: F=3.97, p=.009. CONCLUSIONI: Gli individui richiedenti trattamento per DOCP presentano una importante disfunzione psicosociale che si associa ad alti livelli di perfezionismo e rigidità; tali caratteristiche psicopatologiche potrebbero spiegare perché, nei contesti interpersonali, questi pazienti mantengono la convinzione che le loro aspettative siano violate anche quando nella realtà i loro partner si comportano correttamente. Di contro, soggetti non clinici con DOCP mostrano minore rigidità dei DOCP clinici ed un funzionamento conservato, probabilmente in virtù di elevati livelli di perfezionismo; tuttavia tale caratteristica potrebbe portarli a sottostimare sistematicamente la equità del comportamento altrui negli scambi sociali.
BACKGROUND: Obsessive Compulsive Personality Disorder patients (OCPD) showed different levels of psychosocial disfunctioning. In fact, the core characteristics psychopathological of OCPD such as rigidity and perfectionism might be functional in working environment but it could be disfunctional in social exchanges. Furthermore OCPD functioning, who didn’t request a psychiatric treatment, showed higher levels of psychosocial functioning despite OCPD patients and Mood Disorder which is the frequent comorbidity. No researches clarified if this psychosocial disfunctioning was specific of OCPD or it’s the comorbidity which influenced negatively their funtioning. And also if it is subtended by peculiar mechanisms of processing social rules. OBJECTIVE: The aim is to evaluate the degree of psychosocial functioning in subjects who suffering from DOCP, non-clinical subjects with OCPD (OCPD-NC), healthy controls (HC) and patients suffering from Mood Disorders in remission (MD). And also to examine the answers of these subjects during an experimental paradigm in which social rules are respected and then violated (Cybershape). METHOD: 40 OCPD, 41 OCPD-NC, 22 MD, 81 HC completed questionnaire that assessed their psychosocial global, relational and social-work functioning. The differences between groups have been evaluated through ANOVA univariate with post-hoc Bonferroni test. All the subjects partecipated to Cybershape experiment, which proposed two kind of social interaction: Fair Play (the subjects play with other players who respect the rules) and Rule Violation (the other players systematically break the rules of the game). After each experimental condition the subjects reported the feeling of violation of their expectations and the perception that the rules were violated during the game. Differences between groups in both of these variables were investigated by using the mixed model ANOVA with repeated measures, inserting the Group (OCPD, DUr, OCPD-NC and CS) as a factor “between” subjects and the experimental Condition (Fair Play vs. Rule Violation) RESULTS: OCPD patients showed lower levels of social-work and relational functioning compared to other groups. Instead the OCPD-NC showed a better functioning compared to the MD. As expected OCPD and OCPD-NC appeared more rigid and perfectionist than HC and MD (p<.001) however the OCPD-NC are less rigid than OCPD. In Cybershape, after the Fair Play condition the DOCPs reported a greater feeling that their expectations were violated compared to the CS and the DOCP-NC and this sensation was similar to that experienced in the Rule Violation condition (Group x Condition: F=.28, p=.6). The OCPD-NC reported, instead, a greater violation of the rules during the game compared to the CS and the DOCP, independently from the experimental condition (Group: F = 3.97, p = .009). CONCLUSION: OCPD patients presented an important psychosocial dysfunction that is associated with high levels of perfectionism and rigidity this psychopathological characteristics could explain why these patients maintain, in interpersonal contexts, the belief that their expectations are violated even when in reality their partners behave correctly. On the other hand, OCPD-NC showed less rigidity of OCPD and conserved functioning, probably due to high levels of perfectionism. However, this characteristic could lead them to systematically underestimate the equity of others' behavior in social exchanges.
