Abstract: Background. Il disturbo bipolare è caratterizzato dall’alternarsi di episodi depressivi ed ipo/maniacali. Il disturbo viene classicamente diviso in tipo I e II a seconda della presenza di episodi maniacali o solamente ipomaniacali. Il decorso è cronico e recidivante, tuttavia imprevedibile ed estremamente variabile tra paziente e paziente. Attualmente le linee guida del trattamento si prefiggono di trattare gli episodi ma il decorso, che spesso definisce il funzionamento globale del paziente è perlopiù ignorato, sia come target terapeutico che come specificatore di malattia. Metodi. Abbiamo arruolato n=60 soggetti, in carico al servizio ambulatoriale dal 1990. Sono state valutate le visite psichiatriche a cadenza pressoché bimensile in un intervallo di tempo della durata totale di 135 mesi. Nel periodo di follow-up sono state registrate un totale di 2685 osservazioni, pari a circa 45 valutazioni per ogni paziente. Per ogni valutazione sono state estrapolate dalla cartella clinica le informazioni riguardanti il quadro psicopatologico, eventuali ricoveri, la terapia psicofarmacologica in atto ed eventuali dosaggi ematici di stabilizzatori. Come da indicatori di qualità del servizio oltre il 95% dei pazienti in carico ha ricevuto note psicoeducative riguardanti il disturbo bipolare, tuttavia ventisei soggetti (43.3%) hanno partecipato ad un ciclo strutturato di 21 incontri di Psicoeducazione secondo il modello Colom-Vieta che mira specificatamente a migliorare il decorso. Risultati. Si è osservato, in generale, un effetto del tempo sull'umore (F=2.745; p<.001; eta-squared=.044) con un andamento lineare che suggerisce che tutti i pazienti in carico al servizio, indipendentemente dalle altre variabili, tendono a migliorare e nello specifico a passare meno tempo in depressione (p=.004). Non vi era un effetto del genere (p=.991) né dell'età al momento di presa in carico (p=.199). A conferma dei dati in letteratura i pazienti con diagnosi di Disturbo Bipolare II e NAS trascorrono più tempo con umore depresso (F=5.954; p=.018; eta-squared=.093). Inoltre l’andamento era più favorevole per coloro la cui presa in carico avveniva con un ricovero (F=5.782; p=,019; eta-squared=.091) o con un episodio maniacale (F=4.746; p=.002; eta-squared=.257). Contrariamente a quanto atteso, la Psicoeducazione strutturata non è risultata significativo nel modificare il decorso del disturbo (F=2.009; p=.162). Conclusioni. I risultati ottenuti si sono per gran parte dimostrati in accordo con quelli già presenti in letteratura e suggeriscono la possibilità di riconsiderare il decorso come classificatore del decorso. È possibile che l’effetto della psicoeducazione come intervento strutturato non emerga dalle analisi effettuate poiché l’efficacia del trattamento non è protratta nel tempo. Analisi successive dovranno allargare il campione in esame, considerare il periodo esatto della psicoeducazione strutturata piuttosto che come variabile dicotomica e ricercare specifici cluster di pazienti in base al decorso ed alla risposta al trattamento psicofarmacologico.
Background. Bipolar disorder is characterized by the alternation of depressive and hypo/manic episodes. The disorder is classically divided into types I and II depending on the presence of manic or only hypomanic episodes. The course is chronic and recurrent, yet unpredictable and extremely variable between patient and patient. Currently the treatment guidelines aim to treat the episodes but the course, which often defines the patient's overall functioning, is mostly ignored, both as a therapeutic target and as a disease specifier. Aims. We enrolled n = 60 subjects, in charge of the outpatient service since 1990. Psychiatric examinations were evaluated almost every two months in a time interval of a total duration of 135 months. In the follow-up period a total of 2685 observations were recorded, equal to about 45 evaluations for each patient. For each evaluation, information regarding the psychopathological situation, possible admissions, psychopharmacological therapy in progress and any blood doses of stabilizers were extracted from the clinical record. As for service quality indicators, over 95% of patients in care received psychoeducational notes on bipolar disorder, however twenty-six subjects (43.3%) participated in a structured cycle of 21 psychoeducational meetings according to the Colom-Vieta model that aims to specifically to improve the course. Results. In general, an effect of time on mood was observed (F = 2.745; p <.001; eta-squared = .044) with a linear trend that suggests that all patients in charge of the service, independently of the other variables , tend to improve and specifically spend less time in depression (p = .004). There was no effect of sex (p = .991) nor of age at the time of taking charge (p = .199). To confirm the data in literature, patients diagnosed with Bipolar II and NAS spend more time with depressed mood (F = 5.954; p = .018; eta-squared = .093). Furthermore, the trend was more favorable for those whose care took place with a hospitalization (F = 5.782; p =, 019; eta-squared = .091) or with a manic episode (F = 4.746; p = .002; eta-squared = .257). Contrary to what was expected, structured psychoeducation was not significant in modifying the course of the disorder (F = 2.009; p = .162). Conclusions. The results obtained have largely been demonstrated in agreement with those already present in the literature and suggest the possibility of reconsidering the course as a classifier of the course. It is possible that the effect of psychoeducation as a structured intervention does not emerge from the analyzes carried out since the effectiveness of the treatment is not protracted over time. Subsequent analyzes must broaden the sample under consideration, consider the exact period of structured psychoeducation rather than as a dichotomous variable and search for specific patient clusters based on the course and response to psychopharmacological treatment.
