Abstract: Le cisti ossee subcondrali del cavallo sono causa di zoppia nei cavalli e si presentano in diverse localizzazioni. (Textor et al. 2006) Si ha una tipica zoppia ad intermittenza, ma può presentarsi anche una zoppia acuta subito dopo l’inizio dell’attività fisica, e sono più colpiti gli arti toracici rispetto ai pelvici. (Ohlerth et al. 2008) Le cisti ossee subcondrali nel 40% dei casi comunicano con l’articolazione. (Ohlerth et al. 2008) Molto spesso la lesione tende a presentarsi a livello subcondrale nel punto soggetto a maggior scarico del peso. (McILWRAITH et al. 2010) Molti autori la considerano come una patologia multifattoriale in cui sono implicati vari processi quali: cause genetiche, l’osteocondrosi, i traumi biomeccanici, la dieta, la crescita, gli squilibri ormonali e minerali, la contusione ossea, la pressione esercitata dal liquido sinoviale, e in umana è stato ipotizzato che anche problemi vascolari possano avere un ruolo nello sviluppo della patologia. (Sherlock and Mair 2011b) (Chan et al. 2017) Dal punto di vista istopatologico la cisti ossea subcondrale contiene dell’osso necrotico ed il tessuto fibroso che circonda la cisti produce ossido nitrico, prostaglandine e metalloproteinasi neutre, che comportano il continuo riassorbimento osseo. (Sherlock and Mair 2011b) Dopo aver localizzato il punto con l’anestesia diagnostica si procede con indagini ulteriori quali la radiografia in cui la cisti ossea subcondrale si presenta di forma ovalare, o circolare o conica radiolucente con un bordo di sclerosi radiodenso. Occasionalmente però non si riesce a identificare con la sola radiografia quindi possono essere utilizzati altri strumenti diagnostici quali l’artrografia a contrasto, la TAC in cui la cisti si presenta come un’area ipodensa con attorno un’area iperdensa, la RMN o tramite scintigrafia nucleare. (Sherlock and Mair 2011b) Le cisti ossee subcondrali possono essere trattate sia con terapia conservativa con l’utilizzo di farmaci quali corticosteroidi o FANS in associazione con condroprotettori, (Sherlock and Mair 2011b) oppure tramite terapia chirurgica. Esistono diverse tecniche il cui approccio può essere artroscopico o transosseo quali: il “debridement”, riempimento della cavità cistica con osso spongioso, l’artroplastica a mosaico, innesto di materiali sostitutivi dell’osso, impianti di condrociti autologhi in associazione con fattori di crescita, riempimento con ormone paratiroideo, somministrazione interlesionale artroscopica di steroidi e inserimento di viti transcondilari. (Ortved et al. 2012) La prognosi per il ritorno all’attività va dal 30% all’80% ma dipende dall’età del cavallo, dall’attività sportiva che svolge, dall’estensione dell’area cartilaginea affetta, se si ha o meno concomitante osteoartrite e dal trattamento scelto. (Sherlock and Mair 2011b)
The subchondral horse bone cysts cause lameness in horses and occur in different anatomical locations. (Textor et al. 2006) A typical intermittent lameness might be observed, but also an acute lameness soon after the onset of physical activity has been described. Forelimbs are more affected than your hind limbs. (Ohlerth et al. 2008) Subchondral bone cysts in 40% of cases communicate with the joint. (Ohlerth et al. 2008) Very often the lesion tends to be present at subcondral level at the point subject to greater weight discharge. (McILWRAITH et al. 2010) Many Authors consider it as a multifactorial pathology in which various processes are involved such as: genetic causes, osteochondrosis, biomechanical trauma, diet, growth, hormonal and mineral imbalances, bone contusion, pressure exerted by synovial fluid; in humans it has been hypothesised that vascular problems may also play a role in the development of the disease. (Sherlock and Mair 2011b) (Chan et al. 2017) From the histopathological point of view the subchondral bone cyst contains necrotic bone and the fibrous tissue surrounding the cyst produces nitric oxide, prostaglandins and neutral metalloproteinase, which cause the continuous bone resorption. (Sherlock and Mair 2011b) After the anatomical location of the origin of pain with diagnostic anaesthesia, further investigations are carried out, such as the X-ray in which the subcondral bone cyst may appear as an ovalar, circular or conical radiolucent shadow with an edge of radiopaque sclerosis, when visible. Occasionally, however, it is not possible to identify the lesions with the X-ray alone so other diagnostic tools such as contrast arthrography, CT (Computed Tomography), in which the cyst presents itself as a hypodense area with a hyperdense area around, the RMN or by nuclear scintigraphy are considered. (Sherlock and mair 2011b) Subondral bone cysts can be treated with either conservative therapy with the use of drugs such as corticosteroids or NSAIDs in combination with chondroprotectors, (Sherlock and Mair 2011b) or through surgical therapy. Several are the surgical techniques whose approach can be arthroscopic or transosseous such as: debridement, cystic cavity filling with spongious bone, arthroplasty mosaic, grafting of bone replacement materials, implants of autologous chondrocytes in association with growth factors, reshaping with parathyroid hormone, arthroscopic interlesional administration of steroids and insertion of transcondilar screws. (Ortved et al. 2012) The prognosis for return to activity ranges from 30% to 80% but depends on horse age, sports activity, extent of the affected cartilage area, whether or not you have concurrent osteoarthritis and your chosen treatment. (Sherlock and mair 2011b)
