Approccio clinico, diagnostico e terapeutico alla cardiomiopatia ipertrofica asintomatica del gatto

Abstract

La cardiomiopatia ipertrofica felina (HCM) è una malattia del miocardio ventricolare (in particolare del ventricolo sinistro) in cui si sviluppa un’ipertrofia concentrica del miocardio in assenza di specifiche cause emodinamiche o metaboliche. È la patologia cardiaca acquisita più comune nel gatto e può manifestarsi con diversi fenotipi, tra cui la forma asintomatica. È una malattia ereditaria descritta in diverse razze e negli incroci. Nel Maine Coon e nel Ragdoll è stata anche identificata la mutazione responsabile della sua possibile insorgenza. Nella prima parte della seguente tesi è stata trattata la malattia in generale, prendendo in considerazione epidemiologia, basi genetiche, fisiopatologia, manifestazioni cliniche, diagnosi, terapia e prognosi. La seconda parte della tesi è, invece, dedicata ad un approfondimento sulla forma asintomatica della cardiomiopatia ipertrofica felina. In particolare è incentrata sull’importanza della diagnosi precoce e sui test di screening disponibili per tale malattia, sulla gestione dei soggetti asintomatici, sulla terapia e sulla prognosi di questi gatti.

Feline hypertrophic cardiomyopathy (HCM) is a ventricular myocardial disease (especially of the left ventricle) in which concentric myocardial hypertrophy develops in absence of specific hemodynamic or metabolic causes. It’s the most common acquired cardiac pathology in cats and it can occurs with different phenotypes, including the asymptomatic form. It’s a hereditary disease described in different breeds and crossbreeds. In Maine Coon and Ragdoll was also identified the mutation responsible for its possible onset. In the first part of the following thesis the disease was treated in general, taking into consideration epidemiology, genetic bases, pathophysiology, clinical manifestations, diagnosis, therapy and prognosis. The second part of the thesis is, instead, dedicated to a deepening on the asymptomatic form of feline hypertrophic cardiomyopathy. In particular it focuses on the importance of early diagnosis and on the screening tests available for this disease, on the management of asymptomatic subjects, on the therapy and on the prognosis of these cats
