Abstract: Introduzione Il Disturbo Bipolare è una delle patologie gravi più frequenti, per la quale si è reso necessario strutturare un trattamento organizzato e modulare, in linea con evidenze scientifiche di provata efficacia. Rispetto al passato, oggi si guarda al Disturbo Bipolare con un’attenzione diversa: non ci si limita più a considerare esclusivamente il lato biologico della malattia, ma si sottolinea l’importanza di un’attenta educazione del paziente e della sua famiglia, al fine di migliorare non solo la conoscenza del disturbo, ma anche le strategie necessarie per riconoscere e fronteggiare in maniera adeguata le diverse fasi della malattia. E’ stato dimostrato come la psicoeducazione nel Disturbo Bipolare migliori il decorso clinico, il che si traduce in una riduzione del numero dei ricoveri e delle ricadute. Inoltre, migliora la qualità assistenziale percepita dal paziente, elemento che a sua volta incide in senso positivo sull’alleanza terapeutica e sull’aderenza farmacologica. Grazie alla crescita esponenziale della tecnologia e alla diffusione di smartphone, tablet e PC, la raccolta dei dati in tempo reale è diventata molto più semplice. La valutazione momentanea ecologica è un metodo di acquisizione dei dati relativo al campionamento dell’esperienza, progettato per valutare i dati mentre gli eventi si verificano in tempo reale. E’ stato osservato che un frequente monitoraggio della sintomatologia del Disturbo bipolare, inclusi i sintomi prodromici, consentirebbe ai medici di attuare gli approcci terapeutici più consoni a seconda della situazione e nel modo più tempestivo possibile. Inoltre monitorare l’andamento dell’umore legato ai farmaci, nonché l’eventuale cambiamento dei farmaci, sono importanti per il trattamento. Tra le varie App che consentono di monitorare i sintomi del disturbo bipolare, vi è SIMPLe. Il progetto SIMPLe ha come obiettivo quello di studiare l’utilità dei dispositivi mobili nel follow-up di questi pazienti, fornendo loro uno strumento per la registrazione costante del loro umore, tramite messaggi psicoeducativi. Obiettivi dello studio Il primo obiettivo è stato quello di tradurre dalla lingua Spagnola alla lingua italiana “SIMPLe e-mental health”, un’applicazione che consente di monitorare lo stato d’animo del paziente tramite dei test giornalieri e settimanali. Il secondo obiettivo riguarda la validazione di una nuova metodica per il monitoraggio dei disturbi dell’umore, tramite l’utilizzo di due circomplessi, dove ci siamo chiesti se l’utilizzo all’interno di una popolazione sana venga compreso e se possa essere utilizzato al posto dei questionari classici, riducendo i tempi e guadagnando in accuratezza. Metodi La fase di reclutamento ha coinvolto 19 soggetti (26.3% maschi). Durante la settimana di reclutamento, per due volte al giorno sono stati inviati i due circomplessi e i tre questionari (BDI-II, ASRM e STOQ) ai soggetti che hanno aderito. L’invio degli strumenti di indagine avveniva giornalmente tramite Whatsapp. Risultati Come atteso l’umore nei circomplessi correlava negativamente con BDI, STOQ e fattori estratti dalle scale psicodiagnostiche esploranti le funzioni vegetative (sonno ed appetito). Inaspettate invece erano le correlazioni positive tra i livelli di attivazione ed i fattori estratti inerenti l’affettività (Tristezza e pensiero, Felicità). Per quanto riguarda la differenza di genere, maschi e femmine differivano in “attivazione”, “numero di pensieri” e “velocità di pensieri”, con una maggiore tendenza da parte del sesso maschile a variazioni negative di attivazione e numero di pensieri, probabilmente dovuto alla scarsa omogeneità nella distribuzione dei sessi all’interno del campione (netta predominanza del sesso femminile). Conclusioni Uno dei punti di forza di questo lavoro è rappresentato dalla capacità di esplorazione delle diverse componenti sintomatologiche associate al disturbo bipolare, sul principio delle dimensioni psicopatologiche Kraepeliniane, supportando quindi la possibilità tramite misurazioni EMA di indagare le variazioni inerenti Umore, Ideazione e Psicomotricità con la medesima tecnica. Sulla base di tali osservazioni, pensiamo che l’impiego delle misurazioni tramite l’utilizzo di circomplessi possa garantire un’adeguata esplorazione degli elementi sintomatologici del Disturbo Bipolare, sia nelle fasi intercritiche del disturbo, sia nell’identificazione di variazioni “sotto-soglia” utili nella previsione di prossime fasi episodiche di malattia, riducendo le tempistiche necessarie alla compilazione dei questionari con ripercussioni positive sulla compliance dei Pazienti.
Introduction Bipolar disorder is one of the most frequent serious pathologies, for which it was necessary to structure an organized and modular treatment, in line with scientific evidence of proven effectiveness. With respect to the past, today we look at Bipolar Disorder with a different focus: we no longer limit ourselves to considering exclusively the biological side of the disease, but we stress the importance of a careful education of the patient and his family, in order to to improve not only the knowledge of the disorder, but also the strategies necessary to recognize and cope with the different phases of the disease in an appropriate manner. It has been shown that psychoeducation in Bipolar Disorder improves the clinical course, which results in a reduction in the number of hospitalizations and relapses. Furthermore, it improves the quality of care perceived by the patient, an element which in turn has a positive effect on the therapeutic alliance and on the pharmacological adherence. Thanks to the exponential growth of technology and the spread of smartphones, tablets and PCs, data collection in real time has become much simpler. Ecological momentary assessment is a method of data acquisition related to experience sampling, designed to evaluate data while events occur in real time. It has been observed that frequent monitoring of the symptoms of bipolar disorder, including prodromal symptoms, would allow physicians to implement the most appropriate therapeutic approaches depending on the situation and in the most timely manner possible. Furthermore, monitoring drug-related mood, as well as possible drug changes, are important for treatment. Among the various Apps that monitor the symptoms of bipolar disorder, there is SIMPLe. The SIMPLe project aims to study the usefulness of mobile devices in the follow-up of these patients, providing them with a tool for the constant recording of their mood, through psychoeducational messages.Objectives of the study The first objective was to translate from the Spanish language to the Italian language "SIMPLe e-mental health", an application that allows you to monitor the patient's mood through daily and weekly tests. The second objective concerns the validation of a new method for the monitoring of mood disorders, through the use of two circumventions, where we asked ourselves if the use within a healthy population is understood and if it can be used at the place classic questionnaires, reducing time and gaining accuracy. Methods The recruitment phase involved 19 subjects (26.3% males). During the week of recruitment, two circletions and three questionnaires (BDI-II, ASRM and STOQ) were sent twice a day to the subjects who signed up. The sending of the survey instruments took place daily via Whatsapp. Results As expected mood in the circomplexes correlated negatively with BDI, STOQ and factors extracted from the psychodiagnostic scales exploring the vegetative functions (sleep and appetite). Unexpected, on the other hand, were the positive correlations between the levels of activation and the factors extracted concerning affectivity (Sadness and thought, Happiness). As for the gender difference, males and females differed in "activation", "number of thoughts" and "speed of thoughts", with a greater tendency on the part of male sex to negative variations of activation and number of thoughts, probably due the lack of homogeneity in the distribution of the sexes within the sample (clear predominance of the female sex). Conclusions One of the strengths of this work is the ability to explore the various symptomatic components associated with bipolar disorder, on the principle of Kraepelinian psychopathological dimensions, thus supporting the possibility through EMA measurements to investigate the variations inherent in Mood, Ideation and Psychomotricity with the same technique. On the basis of these observations, we think that the use of measurements through the use of circomplessi can guarantee an adequate exploration of the symptomatic elements of Bipolar Disorder, both in the intercritical phases of the disorder, and in the identification of "sub-threshold" variations. useful in forecasting future episodic phases of illness, reducing the time necessary to complete the questionnaires with positive repercussions on Patients' compliance.
