Abstract: Il tumore al polmone è una patologia di grande importanza clinica per l'alta incidenza e mortalità a livello globale, sono stati sviluppati numerosi farmaci sempre più specifici in base all'istologia, mutazioni e background genetico dei pazienti per rendere più personalizzate le terapie e molti studi clinici sono in corso. Tra le mutazioni genetiche riscontrate in questi pazienti, una piccola percentuale è rappresentata dalle alterazioni del gene MET che codifica per un recettore transmembrana che ha come unico ligando il fattore di crescita degli epatociti (HGF). Questa proteina è implicata della divisione e migrazione cellulare e angiogenesi ed è correlata al cancro quando espressa in maniera animala quale per esempio overespressione, mutazioni, amplificazioni e splicing alternativo dell'esone 14. Ho svolto il dottorato presso il reparto di oncologia del Policlinico Sant'Orsola-Malpighi di Bologna col ruolo di data manager ed ho gestito tre studi clinici con farmaci anti-MET aperti presso il nostro centro. I tre protocolli differivano nei criteri di inclusione e molecole utilizzate ma nel complesso non abbiamo osservato un beneficio clinico che andasse oltre la stabilità di malattia confermata da rivalutazioni radiologiche. In collaborazione con il dipartimento di Anatomia Patologica del nostro Policlinico, stiamo redigendo un database locale di alterazioni del gene MET (amplificazioni e splicing alternativo dell'esone 14) sia per effettuare un pre-screening di pazienti potenzialmente arruolabili negli studi clinici sia per valutare la frequenza di amplificazioni/mutazioni. Fino ad ora abbiamo riscontrato il 27% di amplificazioni Met ed il 9% di mutazioni nei siti di splicing di MET.
Lung cancer is an important pathology due to the globally high incidence and mortality; several drugs have been developed based on the histology, mutations and genetical background of the patients in order to better tailor the therapy. A large number of clinical trials are ongoing to test their efficacy and safety. Among the genetic mutations found in patients with lung cancer, the alterations of gene MET are among the least frequent. This gene encodes for a transmembrane receptor and its unique ligand is HGF (hepatocyte growth factor). MET plays a key role in proliferation, cell motility and angiogenesis; alteration of MET pathway includes overexpression, amplification, mutations and alternative splicing of exon 14. I spent my PhD in the department of oncology of Polyclinic Sant'Orsola-Malpighi, in Bologna; I have been working mainly as data manger of different clinical trials with 3 of them testing new experimental MET-inhibitors. Each trial had different inclusion criteria and active molecules. Overall, we did not observe any clinical benefit in patients enrolled in these trials with the best response being stable disease. In collaboration with the department of Pathologic Anatomy, we created a local database about MET alterations (amplifications and alternative exon 14 splicing) in order to screen patients for clinical trials and to maintain updated local data about the frequency of alterations of this gene. Until now, we found 27% amplified patients and 9% of patients with mutations in MET splicing sites.
