Abstract: L’elaborato che presento pone, come focus centrale della ricerca, l’approccio riabilitativo conosciuto con l’acronimo AOT, ovvero, “Action Observation Treatment”, con lo scopo di valutare la potenziale efficacia di trattamento in pazienti affetti da demenza di Alzheimer di grado lieve. L’obiettivo principale è quello di elaborare delle strategie utili, che possano ritardare il declino funzionale e migliorare pertanto la qualità di vita dei pazienti stessi e di coloro che li assistono. Questo approccio riabilitativo consiste nel far osservare ai pazienti delle azioni finalizzate ad uno scopo compiute da altri individui e coinvolgerli successivamente nella ripetizione del medesimo atto. Si basa su evidenze emerse in ambito neurofisiologico circa il “sistema dei neuroni specchio”, secondo il quale la semplice osservazione di azioni comporta l’attivazione degli stessi circuiti neuronali necessari ad attuarla. In merito all’efficacia di questo trattamento sono stati condotti diversi studi, principalmente riguardanti il recupero delle autonomie, che hanno riscosso particolare successo soprattutto in pazienti parkinsoniani, bambini con paralisi cerebrali infantili ed anche in quei pazienti colpiti da ictus. I risultati significativi relativi agli studi precedenti, dimostrano che esiste un effettivo miglioramento delle attività funzionali nei pazienti che hanno preso parte alla sperimentazione. Lo studio che presento nell’ elaborato, mira ad indagare la possibilità di ottenere il medesimo esito in pazienti affetti da demenza da malattia da Alzheimer di grado lieve, considerato, che anche questi pazienti hanno attività funzionali deficitarie. Rispetto alla procedura AOT consolidata, in questo studio è stata introdotta una variabile cognitiva per rendere il trattamento più adeguato alla tipologia di pazienti inclusi. Previo consenso, i pazienti per lo studio sono stati selezionati da tre differenti strutture, ovvero, il dipartimento di neuroscienze dell’azienda ospedaliero-universitaria di Parma, l’azienda ospedaliera Spedali Civili di Brescia e la Fondazione Santa Lucia di Roma. Il campione reclutato è stato suddiviso e sottoposto al trattamento riabilitativo sperimentale in esame o ad un trattamento che non prevede l’uso di AOT, secondo l’assegnazione randomizzata al braccio sperimentale o al gruppo di controllo. Inoltre, i pazienti selezionati sono stati sottoposti ad una batteria neuropsicologica che prevede la valutazione sia di funzioni prettamente cognitive, che di aspetti funzionali e parametri comportamentali. La valutazione è stata effettuata prima del trattamento riabilitativo, al termine del mese di trattamento ed a distanza di 3 mesi dalla conclusione del trattamento. Dunque, lo studio ha voluto esaminare se AOT induce cambiamenti positivi nel profilo funzionale e cognitivo in pazienti affetti da demenza da Malattia di Alzheimer per migliorarne, in maniera significativa, la qualità della vita e tardare il decorso della patologia.
This thesis presents, as the main focus of research, the rehabilitation approach known as the AOT (Action Observation Treatment), with the aim of evaluating the potential efficacy of the treatment in patients affected by Mild Dementia in Alzheimer's disease. The main goal is to develop useful strategies that can delay the functional decline and thus improve the quality of life of either the patients themselves and those who assist them. This rehabilitation approach consist in letting patients aware of some actions, performed on purpose by other individuals, and then trying to engage them in the repetition of the same act. That procedure is based on some evidences emerged in the neurophysiological field about the "mirror neuron system", according which the mere observation of the actions involves the activation of the neuronal circuits needed to implement it. Several studies have been conducted in order to support the efficacy of this treatment, mainly concerning the recovery of autonomy, which have been especially successful in patients affected by Parkinson disease, in infants affected by cerebral palsy and in those with stroke outcomes. Taking in account the significant results from the previous studies, it was shown that there is a real improvement in functional activity among the patients whom were involved in the experiment. The study I presented in this paper aims to investigate the possibility of obtaining the same outcomes shown in those studies described before, in patients affected by Mild Dementia in Alzheimer's disease, considering that also these patients present disrupted functional activities. Compared to the consolidated AOT procedure, we introduced a a cognitive variable in this study, in order to make the treatment more appropriate to the group of patient involved. The patients were selected from three different structures, in detail from the Neuroscience Department of the Parma University, the Hospital Spedali Civici in Brescia and the Santa Lucia Foundation in Rome. The recruited sample, according to randomized assignment, has been subdivided into two groups; one of those received the experimental rehabilitative treatment and the other one instead received the treatment that does not involve the use of AOT. In addition, the selected patients were subjected to a neuropsychological battery, in order to evaluate different parameters from the groups involved in the study, especially regarding the cognitive and functional aspects and the behavioral parameters. Those evaluations were performed before the rehabilitation treatment, at the end of the month of treatment and after 3 months from the end of the treatment. Therefore, the study aim to examine whether the AOT treatment could induce positive changes in the functional and cognitive profile in patients affected by Mild Dementia in Alzheimer's disease, in order to improve significantly their quality of life and to delay the course of the disease.
