Abstract: I bambini con severi deficit cognitivi provano dolore più spesso e più intensamente dei pari età a sviluppo tipico (Breau et al., 2003): hanno più probabilità di incorrere in dolore accidentale, dolore indotto da idiosincrasie comportamentali riconducibili alla loro condizione e di subire interventi chirurgici (Breau et al., 2003 ; McGrath, 1998). Ciò nonostante, il loro dolore è spesso mal gestito, per due motivi principali: la difficoltà degli operatori sanitari nel riconoscere i loro cues dolorifici, e perché solo da pochi anni si sono rese disponibili scale comportamentali valide e affidabili per comprendere il livello di dolore che il bambino sta vivendo. Ad esempio, la Revised – Face, Legs, Activity, Cry and Consolability Scale (r-FLACC) è stata validata in lingua italiana solo nel 2013 (Di Bari, De Strebecq, Osnaghi e Terzoni, 2013). L’obiettivo di questa ricerca è quello di validare in lingua italiana la checklist NCCPC-PV (Non-Communicating Children’s Pain Checklist-Postoperative Version, Breau et al., 2002) in modo da mettere a disposizione nel nostro Paese uno strumento altrove utilizzato da più di 10 anni, che è risultato valido, affidabile e sensibile nel valutare il dolore nei bambini con deficit cognitivi. In questa tesi sono presentati i dati preliminari, a giugno 2016, dello studio multicentrico che coinvolge Parma, Perugia e Catania. I genitori di nove bambini e adolescenti con gravi deficit comunicativi e intellettivi, di età compresa tra 3 e 18 anni, e i ricercatori hanno osservato per cinque minuti i bambini/ragazzi in cinque epoche diverse, una preoperatoria e quattro postoperatorie, compilando la NCCPC-PV, la r-FLACC e una Scala ad Analoga Visivo (VAS) sull’intensità del dolore percepito. La NCCPC-PV ha dimostrato un’ottima attendibilità interna, in tutte le epoche, sia per i caregiver (α= .83) sia per i ricercatori (α= .88). L’attendibilità inter-rater sui punteggi totali si è dimostrata più che soddisfacente e significativa per tutte le epoche postoperatorie (r= .85-.68), mentre trascurabile e non significativa è risultata quella preoperatoria. Soddisfacente è stata anche la concordanza tra i punteggi degli osservatori nelle sottoscale che si riferiscono ai segnali di natura sociale (Vocale, Sociale e Viso). I punteggi totali della NCCPC-PV erano fortemente correlati con i punteggi post-operatori della r-FLACC dei caregiver (r= .79-.95) e dei ricercatori (r= .82-.95), dimostrando anche una buona validità convergente dello strumento.
Children with severe cognitive impairment experience suffering more intensly and more often (Breau et al., 2003), moreover they are more likely to incur in occasional injuries, caused by their behavioural idiosyncrasy due to their condition, and incur surgery (Breau et al., 2003 ; McGrath, 1998). Nevertheless, pain is often poorly managed, for two main reasons: the first is the difficulty of the medical personnel to recognize the pain clues that these children manifest; the second is that only in recent years have been made available valid and effective behavioural scales to evaluate the degree of pain that the child is experiencing; for example the r-FLACC scale was validated in Italian only in 2013 (Di Bari, De Strebecq, Osnaghi e Terzoni, 2013). The target of this research is to validate the NCCPC-PV checklist in Italian (Non-communicating children’s pain checklist-postoperative version, Breau et al., 2002) in order to make available, in our country, a valid, reliable and sensitive instrument, in use for over 10 years in many other countries, to evaluate pain in children with severe cognitive impairment. Sample and process: the parents (or caregivers) of 9 subjects between age 3 and 18 (10.8±4.5) and researchers have observed for 5 minutes children/young adults, in five different moments: one preoperative and four postoperative. After this period they had to fill in the NCCPC-PV, the r-FLACC and the VAS. Outcomes (results): the NCCPC-PV showed great internal reliability in all five moments, both for the caregivers (α= .83) and for the researchers (α= .88). The inter-rater reliability on the over-all score turned out to be satisfactory and significant for all the postoperative moments (r= .85-.68), while it resulted negligible and not meaningful in the preoperative. A coherence check between observers concerning the NCCPC-PV subscales has demonstrated a satisfactory accord between the observers’ ratings in the subscales regarding the signals having a social nature (Vocal, Social and Face). The complete scores of the NCCPC-PV strongly and significantly linked the r-FLACC complete scores, of both the caregivers (r= .79-.95) and the researchers (r= .82-.95), for the postoperative moments, enabling to define a good convergent validity of the tool.
