Abstract: La centralità della consapevolezza corporea nella formazione del sé trova sostegno non solo nell’ambito psicanalitico, ma anche nell’ambito delle neuroscienze, in cui è stato dimostrato che la sensibilità enterocettiva (sensibilità agli stimoli che hanno origine all’interno del proprio corpo) riveste un ruolo primario nel controllo omeostatico, nella consapevolezza corporea e nell’esperienza dell’emozione. Un recente studio ha evidenziato che un’alta sensibilità enterocettiva si associa anche ad un migliore adattamento in situazioni che richiedono il reclutamento di strategie autonomiche come l’interazione sociale, in linea con la teoria polivagale. Una domanda che pare rilevante, anche a livello clinico, risulta essere la seguente: alterazioni degli aspetti primordiali del Sé, tra questi il controllo enterocettivo, possono contribuire alle anomalie dell’esperienza del sé in ambito psicopatologico? Mentre la presenza di tale associazione sembra essere stata dimostrata in pazienti affetti da schizofrenia, non è ancora chiaro se lo stesso vale per l’Anoressia nervosa, che pure annovera nelle sue manifestazioni sintomatologiche: una visione distorta dell’immagine del proprio corpo con tendenza ad enfatizzarne le dimensioni, difficoltà nel discriminare le sensazioni corporee degli stati emotivi e delle sensazioni viscerali, alterazioni nel funzionamento del sistema nervoso autonomo. Con lo scopo di approfondire quest’ultimo aspetto, il presente studio è volto ad indagare l’esperienza del Sé corporeo e la reattività autonomica in contesti sociali in pazienti anoressiche restrittive. A tal proposito un gruppo di partecipanti sane e un gruppo di anoressiche restrittive sono stati sottoposti a compiti comportamentali in cui sono stati misurati: la sensibilità agli stimoli derivanti dal proprio corpo per mezzo di un compito di percezione del battito cardiaco (Heartbeat perception task); i correlati autonomici in condizioni di riposo e durante un compito di prossemica sociale, misurati per mezzo dell’indice dell’influenza autonomica cardiaca RSA (Respiratory Sinus Arrhytmia), permettente di valutare l’impatto del nervo vago sul cuore. Nella tesi saranno esposti i risultati e le discussioni generali relative alla ricerca.
The centrality of body awareness in the formation of the self finds support not only in the psychoanalytic field, but also in the field of neuroscience, where it has been shown that interoceptive accuracy (sensitivity to stimuli that originate within one's own body) plays a primary role in homeostatic control, the body awareness and the emotional experience. A recent study showed that a high interoceptive accuracy is also associated with better adaptation in situations that require the recruitment of autonomic strategies such as social interaction, in line with the Polyvagal theory. A question that seems relevant, even in the clinical field, is as follows: alterations of the primordial aspects of the self, including the interoceptive control, may contribute to abnormalities of the experience of the self in the field of psychopathology? While the presence of this association seems to have been demonstrated in patients with schizophrenia, it is not yet clear whether the same applies to anorexia, which also includes in its symptomatic manifestations: a distorted view of body image with a tendency to emphasize the size, difficulty in discriminating the bodily sensations of emotional states and visceral sensations, alterations in the functioning of the autonomic nervous system. In order to examine this last aspect, the present study aims to investigate the experience of bodily self and the autonomic reactivity in social settings in anorexic patients. In this regard, a group of healthy participants and a group of anorexics patients were exposed to a behavioral tasks in which were measured: the sensitivity to stimuli arising from their body assested with a heartbeat perception task (Heartbeat perception tasks); autonomic regulation at rest and during a task of social proxemics, measured by RSA (Respiratory Sinus Arrhytmia), in this way it was possible to assess the impact of the vagus nerve on the heart. In the thesis the results of the research will be be shown and a general discussions will follow them.
