Counterfeit medicines and illicit health care products: analysis of active products and chemical profiling

Abstract

During the PhD research, illicit medicines and fake food supplements have been studied, with particular attention in the analysis of active products and development of qualitative/quantitative analytical methods for screening and identification of active ingredients in drug formulation. This activity and the results generated during the 3 years research were useful to give precious information about the trend of illegal market and to complete investigations of AIFA (Italian Medicine Agency) and other Authority (Carabinieri NAS), which necessarily have to be supported by scientific data. Chemical analysis were carried out using high-performance liquid chromatography (HPLC-UV), gas chromatography/mass spectrometry (GC-MS), gas chromatography – flame ionization detector (GC-FID) and more advanced laboratory techniques, such as ultra-performance liquid chromatography/time of flight mass spectrometry (UPLC/TOF-MS). The analytical methods developed and then validated, allowed to qualitatively and quantitatively determine the chemical composition of the drugs (active pharmaceutical ingredients (APIs) as well as other components). During the research, different classes of molecules, mainly present in the illicit market, have been analyzed, in particular the drugs used to treat erectile dysfunction ( ED) such as inhibitors of cGMP-specific phosphodiesterase type 5 (PDE-5), anabolic steroids used for doping as well as cosmetic creams containing corticosteroids and aminoglycosides antibiotics used as skin bleaching. Regarding investigation about illegal PDE-5 inhibitors, no. 21 samples were analyzed as a result of “ad hoc” operations conducted by A.I.F.A related to the market of fake food supplements. Among these samples, no. 7 products were proved to contain sildenafil or tadalafil and in other two samples sildenafil derivatives were identified: thioaildenafil and hydroxythiohomosildenafil; the first of which found for the first time in the Italian market. Regarding cosmetic creams containing corticosteroids and anabolic steroids, active ingredients were monitored in more than 50 samples seized in all the country, providing useful information about trend of illicit market of medicines.

Durante il dottorato di ricerca, farmaci illeciti e falsi integratori alimentari sono stati studiati, con particolare attenzione all’analisi dei principi attivi e lo sviluppo di metodi analitici qualitativi/quantitativi per lo screening e l'identificazione dei principi attivi in varie formulazioni. L’attività e i risultati generati durante la ricerca di tre anni sono stati utili per dare informazioni preziose sulla tendenza del mercato illegale e per completare le indagini di AIFA (Agenzia Italiana del Farmaco) e altre Autorità (Carabinieri NAS), che necessariamente devono essere supportate da dati scientifici. Le analisi chimiche sono state condotte utilizzando la cromatografia liquida ad alta prestazione (HPLC-UV), gas cromatografia/spettrometria di massa (GC-MS), gas cromatografia-rivelatore a ionizzazione di fiamma (GC-FID) e tecniche di laboratorio più avanzate, come cromatografia liquida a ultra prestazioni/spettrometria di massa a “Time of Flight” (UPLC/TOF-MS). I metodi analitici sviluppati e validati hanno permesso di determinare qualitativamente e quantitativamente la composizione chimica dei farmaci (principi attivi farmaceutici (API) nonché altri componenti). Nel corso della ricerca, diverse classi di molecole, principalmente presenti nel mercato illecito, sono state analizzate, in particolare farmaci utilizzati per il trattamento della disfunzione erettile (E.D.) come gli inibitori della fosfodiesterasi di tipo 5 (PDE-5), steroidi anabolizzanti usati come doping e creme cosmetiche contenenti corticosteroidi e antibiotici aminoglicosidi usate come schiarenti della pelle. Per quanto riguarda le indagini sui medicinali inibitori delle PDE-5 illegali, nr. 21 campioni sono stati analizzati in seguito a operazioni “ad hoc” effettuate da AIFA in relazione al mercato dei falsi integratori alimentari. Tra questi campioni, nr. 7 hanno dimostrato di contenere sildenafil o tadalafil e in altri due campioni sono stati identificati derivati del sildenafil: thioaildenafil e hydroxythiohomosildenafil, il primo dei quali trovato per la prima volta nel mercato italiano. Per quanto riguarda le creme cosmetiche contenenti corticosteroidi e gli steroidi anabolizzanti, i principi attivi sono stati monitorati in più di 50 campioni sequestrati in tutto il paese, fornendo informazioni utili sulla tendenza del mercato illegale di farmaci.
