dc.description.abstract | La malattia polmonare interstiziale (ILD) è una complicanza frequente delle miopatie infiammatorie, ed è associata ad alta morbilità e mortalità. Gli anticorpi diretti contro l’aminoacil-tRNA-sintetasi, e tra questi i più comuni anti-Jo1, sono i più forti fattori predittivi di ILD. Con queste premesse, abbiamo analizzato retrospettivamente le immagini acquisite tramite tomografia computerizzata ad alta risoluzione (HRCT) in una coorte di pazienti con poli/dermatomiosite (PM/DM), ILD e positività per anticorpi anti-Jo1. Sono state studiate le scansioni HRCT ottenute al basale e dopo 12 mesi di terapia per valutare eventuali modificazioni associate al trattamento. Sono stati analizzati retrospettivamente 131 soggetti consecutivi con miosite (75 PM, 43 DM, 1 PM amiopatica, 2 sindromi paraneoplastiche, 10 sindromi overlap) che hanno afferito al nostro Dipartimento tra gennaio 1980 e marzo 2011. Sono stati inclusi nello studio i soggetti con diagnosi di PM/DM, con positività per anticorpi anti-Jo1 e ILD evidenziata alla HRCT. L'effetto del trattamento immunosoppressivo (con ciclofosfamide-CYC- o ciclosporina A-CSA- ) associato a corticosteroidi è stato valutato confrontando le scansioni HRCT al basale e dopo 12 mesi per verificarne la stabilità, il miglioramento o peggioramento. Le scansioni HRCT sono state riviste ed interpretate da un radiologo, che non era a conoscenza dei dati clinici. 15 su 131 (11,5%) pazienti consecutivi hanno presentato ILD e positività per anti-Jo-1. Erano tutti affetti da PM, di sesso femminile, con un’ età media di 47 anni (range 19-66 anni) e con durata media dei sintomi di 7,26 mesi (range 2-48 mesi). Alla diagnosi 14 presentavano debolezza muscolare, 12 artralgie/artrite, 9 dispnea e 7 fenomeno di Raynaud. Al basale sono state evidenziate alla HRCT aree a densità ground glass in 8 casi, ispessimento settale in 7 ed honeycombing in 4 pazienti. Dopo 12 mesi in 9 su 15 (60%) la ILD è risultata peggiorata con (intervallo di confidenza esatto-ECI-32-84), in 4 casi la ILD era stabile e 2 pazienti sono stati persi nel follow-up. Non è stato riscontrato miglioramento della ILD in alcun paziente. In particolare, 7 su 15 pazienti sono stati trattati con CsA e in 5 casi (71%) le scansioni HRCT hanno mostrato un peggioramento della ILD dopo 12 mesi (ECI 0,29-0,96). 8 su 15 pazienti sono stati trattati con boli endovenosi di CYC e in 4 (50%) casi la ILD è peggiorata (ECI 16-84). L'evoluzione HRCT nei due gruppi di trattamento non era significativamente diversa. In questo studio la ILD è risultata una comune manifestazione all’esordio nei pazienti con positività per anti-Jo, come riportato in letteratura. Nell’arco di dodici mesi la HRCT ha evidenziato un peggioramento della ILD nella maggior parte di questi pazienti. Nei due gruppi di trattamento con CSA o CYC non è stata osservata alcuna differenza nelle modificazioni del quadro HRCT. | it |
dc.description.abstract | Interstitial lung disease (ILD) is a frequent complication of inflammatory myopathies with high rates of morbidity and mortality. Antibodies against aminoacyl-tRNA-synthetases are the strongest predictive factors in ILD. In this study, we reviewed the literature and we retrospectively analysed high-resolution computed tomography (HRCT) findings in a cohort of 131 consecutive subjects: 75 with polymyositis (PM), 43 with dermatomyositis (DM), one with amyophatic PM, two with paraneoplastic syndromes, and 10 with overlapping syndromes. The inclusion criteria were PM/DM, anti-Jo1 antibody positivity, and HRCT-assessed ILD. The effect of 12 months' treatment with cyclophosphamide (CYC) or cyclosporin A (CsA) plus steroids was assessed by comparing baseline and follow-up HRCT scans for evidence of stability, improvement or worsening. Fifteen patients (11.5%) had ILD and were Jo-1 positive. They were all women with PM, and had a mean age of 47 years and a median duration of symptoms of 7.26 months. At baseline, HRCT showed ground-glass attenuations in eight cases, septal thickening in seven, and honeycombing in four. Twelve months after diagnosis, ILD had worsened in nine patients (60%; exact confidence interval [ECI] 32–84) and was stable in four (two patients were lost to follow-up). Seven of the 15 patients were treated with CsA, and 12-month HRCT revealed a worsening in ILD in five cases (71%; ECI 0.29–0.96); ILD also worsened (ECI 16–84) in four of the eight patients treated with CYC pulses (50%). The evolution of the HRCT findings was not significantly different between the two groups. Our findings confirm that ILD is a common early manifestation in patients with Jo1-positive PM. Over twelve months, HRCT showed worsening ILD in most of our patients, with no difference in the HRCT changes between those treated with CYC or CsA. | it |
