dc.description.abstractLe anomalie cromosomiche sono la causa principale di mortalità perinatale e di handicap neonatale. La ricerca di un test di screening per le anomalie cromosomiche, a basso costo, privo di rischi e proponibile a tutte le donne in epoca precoce di gravidanza, ha portato alla diffusione nella pratica clinica del Test combinato nel primo trimestre di gravidanza, che consente di identificare i feti affetti da trisomia 21 con una sensibilità del 90% per un numero di falsi positivi del 5%.Dallo studio condotto presso il Servizio "Spazio Salute Donna " dell'Azienda Sanitaria Locale di Parma, è emerso che la distribuzione dei valori della translucenza nucale nel gruppo di gravide esaminate è in accordo con quanto atteso dai dati forniti dalla Fetal Medicine Foundation con una incidenza di falsi positivi del 6%. E' stato invece riscontrato un numero eccessivo di falsi positivi derivati dall'analisi dei marcatori biochimici, dato che pone una riflessione sull'importanza di contenere il numero dei falsi positivi al fine di ridurre il numero delle procedure invasive, con i rischi di abortività che queste comportano. Le pazienti dello studio hanno dichiarato inoltre, dal questionario proposto, di avere ricevuto un'adeguata informazione prima dell'esecuzione dell'esame, ed hanno manifestatoun alto gradimento del test combinato, con oltre il 97% di donne che ripeterebbe il test in una successiva gravidanza.Il test combinato rappresenta quindi un importante test di screening delle anomalie cromosomiche, che consente di aumentare in maniera significativa il numero di donne che preferiscono eseguire indagini di screening nella fase precoce di gravidanza, riducendo il ricorso alla diagnosi prenatale invasiva.it
dc.description.abstractChromosomal abnormalities are the leading cause of perinatal mortality and neonatal handicap.The search for a screening test for chromosomal abnormalities, low cost, without risk, and proposes that all women in early pregnancy period, led to the spread in the clinical practice of combined test in the first trimester of pregnancy, which allows identify fetuses with trisomy 21 with a sensitivity of 90% for a number false positive of 5%.
